California Licensed Telephonic Nurse Case Manager

Job Summary

To coordinate comprehensive patient care for high-risk individuals, the full-time California Licensed Telephonic Nurse Case Manager will evaluate treatment plans, manage resources, and collaborate with patients, families, and medical professionals in a remote setting.

Key responsibilities

• Establish collaborative relationships as an intermediary between clients, patients, employers, providers, and attorneys
• Assess and implement care plans while promoting quality, cost-effective care to facilitate optimal recovery and return to work
• Document all case management activities and maintain organized clinical records of interactions related to each claim

Required qualifications

• Current RN License, preferably in a compact state, with a California RN license being a plus
• At least 3 years of Telephonic Case Management experience preferred
• Experience with Workers' Compensation or Disability Management
• Strong computer skills, particularly in Microsoft Suite (Outlook, Excel, Word, etc.)
• Associate degree; BSN, CCM, or other relevant certifications are a plus
