To support the coordination of member transitions in the community, the full-time California Licensed Transition Specialist (LVN/LPN) will manage care plans, educate members on services, and collaborate with healthcare teams, all while working remotely in California.
Key responsibilities:
- Supports care coordination teams in developing effective transition plans for community members
- Assists with member transitions based on enrollment or care service needs
- Tracks transition metrics and ensures proper documentation of care transitions
Required qualifications:
- Current California LVN/LPN license
- Bachelor's degree with 2-4 years of related experience, or equivalent experience
- Experience in charting, case management, discharge planning, and telephonic care coordination
