California Patient Care Coordinator

Job Summary

Focusing on telephonic outreach, the full-time California Patient Care Coordinator will engage prospective CalAIM members, educate them on available services, and facilitate enrollment into Enhanced Care Management and Community Supports programs while working remotely from California.

Key Responsibilities

• Conduct high-volume outbound telephone outreach to prospective CalAIM members and explain available services
• Screen potential participants for program eligibility and schedule enrollment appointments
• Document outreach activities and maintain productivity standards related to call volume and enrollment goals

Required Qualifications

• High school diploma or equivalent
• Minimum of one year of patient-centered care experience in healthcare outreach or related fields
• Proficiency with Microsoft Office and web-based applications
• Ability to manage a high volume of outbound calls while maintaining professionalism and empathy
• Experience working with Medicaid/Medi-Cal populations is preferred
