California RN Care Navigator

Job Summary

To facilitate high-quality kidney health management, the full-time California RN Care Navigator will manage a caseload of patients remotely, focusing on care coordination, member education, and case management activities.

Key responsibilities

• Handle inbound and outbound calls to provide exceptional service and support to members
• Educate members on kidney health, treatment options, and engage them in HealthMap's Kidney Health Program
• Collaborate with interdisciplinary care teams to coordinate care and address patient needs effectively

Required qualifications

• Active, unrestricted RN license required
• Bachelor's degree required; or 6+ years of RN experience including 3+ years in case management may be considered in lieu of degree
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience working with nephrology or cardiology patients preferred
