California RN Case Manager

Job Summary

Compassionate and experienced, the full-time Telephonic RN Case Manager for the Special Needs Plan will provide telephonic case management to medically complex and chronically ill members, conduct comprehensive health assessments, and coordinate care, all while working remotely from California.

Key responsibilities

• Provide telephonic case management to medically complex and chronically ill members
• Conduct comprehensive health assessments and create individualized care plans
• Coordinate care with internal and external partners, including physicians and specialists

Required qualifications

• Active, unrestricted RN license in California (Non-Compact)
• Minimum 2 years of clinical nursing experience
• At least 1 year of case management experience
• Proficiency with Microsoft Office (Word, Excel, Outlook)
• Bilingual candidates preferred (Spanish, Korean, Mandarin, etc.)
