Job Summary
Supporting members throughout their care journey, the full-time Cardiovascular Clinical Guide will conduct comprehensive hypertension assessments, provide lifestyle counseling, and empower members to achieve their health goals in a remote setting.
Key responsibilities
- Screen members for eligibility in the Hypertension Clinic and conduct comprehensive assessments
- Deliver intensive lifestyle and nutrition counseling focused on heart health and behavior change
- Coordinate multidisciplinary care and serve as the primary point of contact for members throughout their care journey
Required qualifications
- Experience as a Medical Assistant or in a clinical/healthcare setting
- At least 2 years of work experience in longitudinal chronic disease management
- Ability to utilize motivational interviewing skills to influence positive behavioral changes
- Experience in providing lifestyle coaching and support for chronic conditions
- Familiarity with hypertension management and related health education
