Care Connector

Job Summary

Supporting health management programs, the part-time Care Connector will interact with members and providers to implement interventions, conduct outreach calls, and assist members in accessing care and community resources while working remotely from the Washington, D.C. area.

Key responsibilities

• Conduct inbound and outreach calls to screen members and collect data according to established protocols
• Identify members' needs and link them to community resources and healthcare access
• Document member interactions accurately in the electronic care management platform

Required qualifications

• High School Diploma or GED required; Associate degree preferred
• Minimum of 2 years of work experience in a healthcare environment required
• At least 1 year of telephonic customer service experience, preferably in healthcare
• Medical terminology experience preferred
• Proficiency in MS Office applications and experience with electronic medical record systems preferred
