Care Connector

Job Summary

Supporting the daily operations of integrated care management programs, the remote Care Connector will process calls from members and providers, identify care gaps, and assist members in accessing care through plan benefits and community resources.

Key responsibilities

• Process calls from members and providers in a contact center environment
• Identify members with care gaps and assist them in accessing necessary care
• Provide educational materials and strategies to enhance healthcare adherence under clinical staff direction

Required qualifications

• High School Diploma or equivalent required
• Minimum two years of recent customer service experience
• Experience in member service or customer service telephone roles, preferably in managed care
• Medical terminology experience preferred
• Preferred training or certification as a Medical Assistant, Home Health Aide, or Nursing Assistant
