To support a growing digital health platform, the full-time Care Coordinator will manage a full patient caseload through app chat, ensuring timely and professional communication while coordinating care and resolving issues remotely.
Key responsibilities
- Manage patient interactions primarily through written chat, providing clear and supportive messages
- Own response times and quality metrics, consistently meeting service level agreements (SLAs)
- Coordinate with pharmacies and labs to troubleshoot prescription and delivery issues
Required qualifications
- At least 2 years of experience in a chat-based Care Coordinator or similar role
- Exceptional writing skills, with a professional and warm tone in communication
- Ability to work in a fast-paced, metric-driven environment with clear SLAs
