Care Coordinator

Job Summary

Supporting transitions of care in a full-time remote capacity, the Care Coordinator will manage patient outreach, coordinate post-discharge follow-ups, and assist with operational tasks to ensure seamless patient care.

Key responsibilities

• Reaching out to patients while inpatient to introduce the transitions of care program and communicate post-discharge follow-up plans
• Scheduling post-discharge follow-ups with primary care and specialty providers
• Coordinating lab testing and imaging, tracking results to ensure review by the ordering clinician

Required qualifications

• 2+ years of patient-facing and care coordination experience within healthcare, managed care, home health, or community health settings
• Experience working in a telemedicine environment
• Ability to thrive in a fast-paced, high-energy organization
• Strong communication skills to interact effectively with various stakeholders
• Entrepreneurial self-starter who takes initiative and thrives as a collaborative team player
