Care Coordinator

Job Summary

Supporting patients across brain and mental health services, the full-time remote Care Coordinator will manage patient intake, coordinate care, and ensure compliance with clinical standards while collaborating with interdisciplinary teams.

Key Responsibilities

• Conduct patient screening calls and assist with intake processes, ensuring timely and compassionate care
• Coordinate treatment planning and appointment scheduling while managing administrative workflows and patient follow-up
• Maintain compliance with documentation standards and participate in team meetings to uphold high care standards

Required Qualifications

• Compliance with state board requirements, including fingerprinting and application submission
• Proficiency with telehealth platforms and remote clinical workflows
• Experience in a mental health clinic or psychiatric setting with knowledge of patient workflows
• Experience in a fast-paced call center environment requiring immediate follow-up
• Ability to work independently and collaboratively in a dynamic team environment; Spanish speaking is a plus
