Job Summary
To support patients in the Pacific Time Zone, the fully remote Care Coordinator will manage patient engagement, care coordination, and resource navigation, ensuring comprehensive support and improved health outcomes for individuals living with kidney disease.
Key responsibilities
- Perform telephonic outreach to enroll and engage patients, serving as their primary point of contact
- Develop and implement care plans in collaboration with the interdisciplinary care team, addressing patients' unique needs

Required qualifications
- 3 years of experience in healthcare coordination or community health work
