Care Coordinator DSNP

Job Summary

Providing administrative and coordination support to the interdisciplinary Care Team, the full-time Care Coordinator DSNP will manage member outreach, conduct assessments, and facilitate access to services while working remotely.

Key responsibilities

• Welcome and orient new members to the DSNP program, conducting outreach and assessments
• Support the development and implementation of each member's Individualized Care Plan (ICP) and coordinate access to necessary services and resources
• Educate members and caregivers on plan benefits and follow up to address barriers to care

Required qualifications

• 2+ years of experience in care coordination, customer service, or healthcare
• Experience in a managed care or HMO setting is strongly preferred
• Understanding of the needs of older adults and disadvantaged populations
• Bilingual (Spanish) is required
• Bachelor's degree in social or health sciences is preferred
