Care Coordinator - NV Licensed

Job Summary

Supporting patients in a remote full-time capacity, the Care Coordinator - NV Licensed will manage patient intake, treatment coordination, and risk management while collaborating with interdisciplinary care teams to ensure seamless care delivery in brain and mental health services.

Key Responsibilities

• Conduct patient screening calls and assist with intake processes, ensuring accurate documentation and navigation of insurance coverage
• Coordinate treatment planning and appointment scheduling, collaborating with clinical teams and external partnerships
• Maintain compliance with documentation standards and promote a patient-centered approach throughout the care journey

Required Qualifications

• Compliance with state board requirements, including fingerprinting and application submission
• Proficiency with telehealth platforms and remote clinical workflows
• Experience in a mental health clinic or psychiatric setting with knowledge of patient workflows
• Background in a fast-paced call center environment with immediate follow-up on multi-channel lead capture
• Ability to work independently and contribute effectively in a collaborative team environment; Spanish-speaking is a plus
