Care Coordinator - Wisconsin Licensed

Job Summary

Serving as a patient advocate, the full-time remote Care Coordinator - Wisconsin Licensed will manage patient experiences from preadmission through treatment, coordinating care and maintaining communication with patients and interdisciplinary teams.

Key responsibilities

• Support patient experience by relaying program recommendations, coordinating admissions, and addressing inquiries from patients and families
• Ensure timely follow-up with patients and facilitate internal transfers between sites by communicating pertinent information
• Develop working relationships with interdisciplinary team members to enhance communication regarding patient care

Required qualifications

• High school diploma required; bachelor's degree preferred
• Two years of customer service and relevant experience, preferably in a healthcare setting
• Proficiency with computers and experience with software platforms and EMRs
• Knowledge of medical/psychiatric/chemical dependency diagnosis and treatment processes preferred
• Basic Life Support (BLS) and Nonviolent Crisis Intervention (NCI) certifications required
