Job Summary

To enhance the patient experience, the full-time Care Guide will build long-term relationships with members, providing concierge-level service, care navigation, and health coaching in a remote setting.

Key responsibilities

• Manage patient relationships by conducting proactive outreach, regular check-ins, and maintaining accurate documentation of interactions
• Utilize motivational interviewing techniques to support behavior change and reinforce care plans for chronic conditions
• Coordinate care by partnering with the Care Pod, scheduling appointments, and addressing barriers to access

Required qualifications

• Minimum of 3 years of experience in outpatient care, virtual care, or health coaching
• Experience using motivational interviewing or health coaching techniques
• Proficiency with EHRs and digital communication tools
• High emotional intelligence and professionalism
• Ability to manage multiple patient relationships and workflows effectively
