Care Management Associate

Job Summary

Supporting comprehensive coordination of medical services, the full-time Care Management Associate will manage Care Team intake, screening, and the implementation of care plans while working remotely.

Key responsibilities

• Conduct initial reviews and triage Care Team tasks, identifying the principal reasons for admission and necessary medical services
• Coordinate and arrange healthcare service delivery under the direction of clinical staff, ensuring effective utilization of resources
• Utilize various systems to build and enter member information, monitor cases, and promote communication among healthcare providers and team members

Required qualifications

• 2-4 years of experience in the healthcare field, including medical billing and coding
• Experience in a call center or customer service environment
• Ability to work independently while collaborating with clinical teams
• Proficiency in Microsoft Office 365 applications and navigating multiple systems simultaneously
• High School Diploma or G.E.D
