Care Management Associate

Job Summary

Supporting comprehensive coordination of medical services, the full-time Care Management Associate will manage Care Team intake, screening, and implementation of care plans while working remotely on a weekend schedule from 7 AM to 5:30 PM, Saturday through Tuesday.

Key responsibilities

• Conduct initial review and triage of Care Teams tasks, identifying patient needs and coordinating appropriate medical services
• Support the development and implementation of care plans, ensuring effective utilization of health services
• Promote communication among healthcare providers and team members to enhance medical management services

Required qualifications

• 2-4 years of experience in a healthcare setting, including medical billing and coding
• Experience in a call center or customer service environment
• High School Diploma or G.E.D
• Ability to work independently and meet deliverables in a fast-paced environment
• Proficiency in Microsoft Office 365 applications and ability to navigate multiple systems simultaneously
