Job Summary
Supporting administrative care management activities, the full-time Care Management Support Coordinator will perform outreach, answer inbound calls, and schedule services for Medicaid LTSS members aged 21 and older, working remotely from various locations.
Key responsibilities:
- Conduct outreach to members to assist with care plan next steps and connect them to health plan and community resources
- Serve as the front-line support for member and provider inquiries, explaining care plan procedures and protocols
- Document and maintain non-clinical member records in compliance with state and regulatory requirements
Required qualifications:
- High School diploma or GED
- 1 - 2 years of related experience in a remote call center environment
- 2+ years of experience in member or provider outreach in a fully outbound call setting
- 2+ years of experience processing healthcare or insurance authorizations
- Experience supporting adults aged 21+ receiving Long-Term Services and Supports (LTSS) preferred
