Care Management Support Specialist

Job Summary

To support a growing ambulatory network, the full-time Care Management Support Specialist will triage referrals, connect patients to appropriate care teams, and manage reporting requirements while working remotely with occasional travel to Massachusetts locations.

Key responsibilities

• Triage referrals to the Ambulatory Care Management Department using the EPIC Compass Rose platform
• Maintain up-to-date referral work queues and complete documentation in EPIC and other databases accurately and timely
• Act as a primary contact for internal and external collaterals, fostering professional relationships and responding to requests promptly

Required qualifications

• High School Diploma or equivalent
• Two (2) years of experience in a clinical setting, such as a Nursing Unit or Care Management Department
• Proficiency in Microsoft Applications, particularly Excel
• Experience working in the human services field is preferred
• College degree or Medical Assistant National Certification is a plus
