Job Summary
Managing an assigned caseload of patients, the remote Care Manager will provide personalized education and ongoing support for individuals undergoing Positive Airway Pressure (PAP) therapy, ensuring adherence and compliance with insurance requirements while working Monday through Friday from 12:30 PM to 9:00 PM ET.
Key responsibilities
- Provide proactive outreach and education to patients throughout their PAP therapy journey
- Monitor therapy adherence and identify patients at risk of noncompliance, offering timely interventions
- Build strong relationships with patients through active listening and personalized coaching
Required qualifications
- Bachelor's degree or relevant clinical/customer service experience, preferably in healthcare
- Knowledge of PAP therapy and common troubleshooting techniques; sleep medicine experience preferred
- Strong organizational skills with the ability to manage multiple priorities in a fast-paced environment
- Self-motivated and independent, demonstrating initiative and accountability
- Experience with motivational interviewing or patient coaching preferred
