Care Navigator

Job Summary

To support patients needing care outside of Crossover's clinics, the part-time PRN Care Navigator will manage the referral process, including scheduling appointments, maintaining specialist directories, and ensuring continuity of care through effective communication with patients and providers.

Key responsibilities

• Process daily referrals to secondary care and maintain high-quality directories of specialists and community services
• Ensure timely communication and documentation of referrals to facilitate continuity of care
• Serve as the primary contact between patients and specialists, providing updates to the clinical team as necessary

Required qualifications

• At least 3 years of applicable experience, preferably in a Primary Care setting
• High school diploma or GED required
• Proficient in Google spreadsheets and/or Excel, as well as database management and data tracking
