Care Navigator

Job Summary

Guiding older adults and their caregivers through the intake process, the full-time remote Care Navigator will provide patient support, care coordination, and emotional guidance, ensuring a compassionate and seamless experience as they enter mental health care.

Key responsibilities

• Serve as the first point of contact for patients and families, facilitating the intake process with empathy and professionalism
• Coordinate care by matching patients with appropriate therapists and resources, ensuring they feel understood and supported
• Manage multiple patient cases simultaneously, maintaining organization and attention to detail throughout the process

Required qualifications

• Strong interest in mental health and healthcare access for older adults
• Experience in healthcare, customer service, or administrative support is preferred but not required
• Proficiency with technology, including Google Workspace and EHR platforms
• Excellent organizational skills to handle multiple patients at various stages of the intake process
• Empathetic communication skills that foster trust and reassurance during difficult conversations
