Care Navigator

Job Summary

To support patient care, the full-time remote Care Navigator will manage outbound and inbound calls with patients and physician offices, assisting in scheduling follow-up appointments after hospital discharge.

Key responsibilities

• Contact patients recently discharged from the hospital to assist with scheduling follow-up appointments
• Make calls to physician offices to schedule appointments for patients who accept services
• Utilize computer systems to read charts, fax records, and document patient scheduling preferences

Required qualifications

• Minimum 18 years old with a high school diploma or equivalent
• Preferred completion of at least 1 year of college coursework
• Ability to troubleshoot basic computer issues
• Comfortable navigating multiple computer applications and windows
• Self-motivated and responsible individual with a people-first mentality
