Care Navigator

Job Summary

To support a growing healthcare initiative, the remote Care Navigator will engage patients, coordinate their care across various settings, and connect them to necessary resources while working within a flexible schedule.

Key responsibilities

• Conduct proactive outreach to schedule patient visits and ensure timely preventative care
• Collaborate with Nurse Practitioners to manage patient panels and coordinate care follow-up
• Maintain accurate documentation of patient interactions and care coordination activities

Required qualifications

• 1-2 years of post-high school education or an associate's degree; bachelor's degree in a related field is a plus
• 1-3 years of experience in a patient-facing healthcare role
• Familiarity with medical terminology and chronic disease management for older adults
• Strong organizational skills and ability to multitask in a fast-paced environment
• Professionalism in communication and demeanor, with sensitivity to HIPAA compliance
