Care Navigator

Job Summary

To support a growing healthcare practice, the remote Care Navigator will manage patient outreach and engagement, coordinate care with Nurse Practitioners, and ensure compliance with documentation while working part-time in the Mountain time zone from 7 AM to 4 PM MST.

Key responsibilities

• Conduct proactive outreach to schedule patient visits and ensure timely preventative care
• Coordinate patient care and manage follow-ups post-discharge to reduce readmission risks
• Maintain accurate documentation of patient interactions and care coordination activities

Required qualifications

• 1-2 years of post-high school education or an associate's degree; bachelor's degree in a related field is a plus
• 1-3 years of experience in a patient-facing healthcare role
• Familiarity with medical terminology and chronic disease management in older adults
• Outstanding phone skills with a focus on HIPAA compliance
• Organized and detail-oriented with the ability to multitask in a fast-paced environment
