Care Navigator

Job Summary

Connecting with patients eligible for a virtual musculoskeletal program, the full-time Care Navigator will provide support from enrollment through program completion in a remote environment, ensuring a smooth care journey by coordinating with the care team and addressing patient needs.

Key responsibilities

• Engage with patients through inbound and outbound communications to explain program benefits and guide them through the enrollment process
• Act as the primary point of contact for patients, coordinating care and ensuring appointments are scheduled and kept
• Maintain accurate documentation and communicate effectively about patient care and program expectations

Required qualifications

• Two or more years of experience in a patient-facing or care coordination role
• Proficiency in CRM and EHR systems with a commitment to thorough documentation
• Strong communication skills and ability to build rapport with patients and team members
• Organizational skills to manage multiple patients and priorities effectively
• Motivated by metrics and results, demonstrating a strong work ethic and adaptability in a fast-paced environment
