Care Navigator

Job Summary

To support a new Care Navigation & Triage program, the part-time contract Care Navigator will coordinate patient referrals from hospitals to ensure timely follow-up care and connect patients with necessary resources in a remote setting.

Key responsibilities

• Receive and manage referrals from partner hospitals, acting as the main contact for discharge planners and care teams
• Schedule patients with Clinical Assessors and facilitate their appointments with appropriate care providers or community resources
• Follow up on patient attendance, provide practical support, and document all interactions while adhering to privacy and confidentiality requirements

Required qualifications

• Minimum of 2 years of experience in behavioral health care coordination, case management, or hospital discharge planning
• Comfortable working with recently discharged patients and recognizing when to involve clinical support
• Highly organized with the ability to manage a high volume of referrals and follow-ups
• Strong communication skills, with a warm approach to interacting with patients and healthcare providers
• Familiarity with insurance and community resources in Missouri or Virginia is a plus
