Job Summary
Providing guidance and support to clients and their families, the full-time Care Navigator will assist in navigating behavioral health resources, educate clients on their options, and create tailored plans to enhance well-being in a fully remote environment.
Key responsibilities

Required qualifications
- Bachelor's degree in healthcare administration, social work, nursing, or a related field
- 2-3 years of experience in care coordination, case management, or client advocacy
- Knowledge of healthcare systems, medical terminology, and community resources
- Proficiency in computer applications, including electronic health records and Microsoft Office Suite
- Completion of or enrollment in a Patient Navigator Training Course
