Care Services Coordinator

Job Summary

Providing comprehensive support to individuals and families affected by ALS, the part-time Care Services Coordinator will manage inquiries, coordinate care plans, and connect clients with resources while working remotely from Alaska.

Key responsibilities

• Respond to inquiries from individuals living with ALS and their families regarding resources and support
• Conduct needs assessments and care consultations in various settings, including virtual and in-person visits
• Document interactions and maintain engagement with clients using Salesforce and other tools

Required qualifications

• Bachelor's degree in social work, nursing, public health, or a related field; relevant experience may be considered
• Associate's degree with 3+ years of experience in a similar role may be acceptable
• Knowledge of Medicare, Medicaid, patient assessments, and case management preferred
• Excellent computer skills, including proficiency in Microsoft Office and Salesforce
• Ability to travel locally and work occasional evenings or weekends for Association events
