Care Team Operations Associate

Job Summary

Supporting clinical and social care management needs, the full-time remote Care Team Operations Associate will coordinate home health services, manage referrals, and communicate with members and external agencies to ensure effective care delivery.

Key responsibilities

• Coordinates home health, insurance, vendors, and other social assistance services for members
• Initiates and manages referrals based on member needs, ensuring effective communication throughout the process
• Uses analytical platforms to generate and communicate weekly reporting and planning to support clinical care

Required qualifications

• 1 year of administrative experience in scheduling, coordination tasks, or maintaining accurate records
• Experience in connecting members with medical services and social assistance
• Familiarity with member benefits platforms and Medicaid support processes
• Ability to take notes and follow up on commitments during team meetings
