Job Summary
To support patient transitions effectively, the full-time Care Transition Coordinator will work remotely to manage referral coordination, review medical records for eligibility, and ensure patients receive appropriate care in a timely manner.
Key responsibilities
- Review medical records against clinical criteria to determine care eligibility
- Document and track patient referrals in company systems, collaborating with Care Transitions Nurses
- Educate patients and families about Home Health, Hospice, and Palliative Care services
Required qualifications
- Call center experience in a healthcare setting
- Minimum of 1 year of healthcare or healthcare call center experience
- Associate degree preferred
- Strong computer skills and familiarity with multiple software applications
- Knowledge of Home Health, Hospice, and Palliative Care services
