Job Summary
Supporting members transitioning from inpatient care to home or other settings, the full-time Care Transition Specialist (PA/NJ/DE) will engage in proactive outreach, education, and coordination of services to ensure continuity of care and promote positive health outcomes in a fully remote role.
Key responsibilities
- Contact members post-discharge to review instructions, identify needs, and escalate issues to the clinical team
- Assist with scheduling follow-up visits and facilitate referrals to necessary services and resources
- Document member interactions and monitor high-risk members, escalating concerns to clinical case managers as needed
Required qualifications
- Bachelor's degree in health sciences, psychology, social work, nursing, public health, or a related field
- 1-2 years of experience in healthcare or member-facing roles, particularly in case management and outreach
- Knowledge of managed care principles, discharge planning, and community resources
- Medical Assistant certificate preferred
- Ability to work independently and manage multiple priorities
