Case Manager
Job Summary
Seeking a compassionate and highly organized Case Manager, this fully remote position will monitor client progress, ensure adherence to treatment plans, and provide ongoing support to individuals recovering from Substance Use Disorder.
Key responsibilities
- Actively review client activity and monitor progress against individualized treatment plans
- Identify barriers to client engagement and collaborate with the clinical team to develop interventions
- Coordinate client appointments and assist with accessing community resources and referrals
Required qualifications
- High school graduate or equivalent required; associate degree preferred
- 1-3 years of experience in a behavioral health setting preferred
- Strong understanding of the healthcare system, including medical insurance
- Fluency in English and Spanish is preferred
- Prior experience utilizing an EHR/EMR strongly preferred
