Certified Diabetes Educator
Job Summary
To support a growing practice, the part-time Certified Diabetes Educator will guide optimal diabetes care, manage medication coordination, and educate patients on lifestyle changes while working remotely on a contract basis for 8-15 hours per week.
Key responsibilities:
- Serve as Quality Coordinator, overseeing Diabetes Self-Management Education and Support services and ensuring evidence-based practices
- Educate patients on lifestyle changes related to diabetes management, including nutrition and medication management
- Facilitate diabetes and nutrition education classes and provide family support as needed
Required qualifications:
- Bachelor's Degree in Nutrition or a related field of human services from an accredited college or university
- Certified Diabetes Care and Education Specialist Credential
- Registered Dietitian or Registered Nurse credentials
- Three years of clinical experience preferred
- Experience facilitating behavior change in the area of eating disorders/mental health preferred
