Job Summary
As a full-time Certified Nursing Assistant (CNA) Care Advocate working remotely, the candidate will support Medicare patients by helping them understand their care plans, schedule appointments, and navigate healthcare resources while maintaining accurate documentation and compliance with privacy standards.
Key responsibilities
- Serve as the primary point of contact for patients post-intake, guiding them through their care plans and Medicare benefits
- Resolve navigation issues related to appointments, referrals, and provider communications while identifying and connecting patients to social support resources
- Document all patient interactions and support activities accurately and in a timely manner
Required qualifications
- Certified Nursing Assistant (CNA) certification or equivalent hands-on patient-care experience
- 2+ years of patient- or community-facing support experience in roles such as care coordination or health coaching
- Working knowledge of the U.S. healthcare system and Medicare
- Ability to work independently while managing a caseload and maintaining documentation
- Reliable computer and internet access for patient calls
