Chief Population Health Officer

Job Summary

Providing strategic and clinical leadership in a remote, full-time capacity, the Chief Population Health Officer will oversee value-based care initiatives, develop provider contracts, and drive improvements in clinical outcomes and financial performance.

Key responsibilities

• Provide executive leadership and expertise across value-based care and population health initiatives
• Develop and oversee strategies for provider contracts, reimbursement models, and clinical quality programs
• Lead the expansion of clinically integrated networks and collaborate with health system leadership to implement sustainable programs

Required qualifications

• Significant executive-level experience in value-based care and population health within the provider ecosystem
• Deep expertise in value-based care contracting and provider reimbursement models
• Proven experience in building and overseeing clinically integrated networks (CINs)
• Strong understanding of clinical, operational, and financial dimensions of value-based care
• Demonstrated ability to work with senior executives and healthcare stakeholders
