Job Summary
To enhance medication adherence and improve health outcomes, the remote Clinical Pharmacist - TX, TN, AL, PA will lead HEDIS initiatives, conduct patient engagement sessions, and analyze data to implement effective interventions in a full-time capacity.
Key Responsibilities
- Lead efforts to improve HEDIS medication adherence and pharmacy quality measures
- Conduct medication therapy management sessions to address patient barriers and provide education
- Monitor HEDIS performance metrics and generate reports on intervention effectiveness
Required Qualifications
- Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy
- Active pharmacist license, with a willingness to obtain licenses in TX, TN, AL, and PA
- Minimum of 2 years of experience in HEDIS-related medication adherence or quality improvement
- Strong knowledge of HEDIS measures related to medication adherence
- Proficiency in using EHR systems and data analysis tools
