Job Summary
To enhance medication adherence and improve health outcomes, the full-time remote Clinical Pharmacist - TX, TN, AL, PA will lead HEDIS quality initiatives, conduct patient engagement sessions, and analyze adherence data while collaborating with healthcare providers.
Key Responsibilities
- Lead efforts to improve HEDIS medication adherence and analyze data to implement targeted interventions
- Conduct medication therapy management sessions and utilize motivational interviewing techniques to engage patients
- Monitor HEDIS performance metrics and provide updates to leadership on progress towards goals
Required Qualifications
- Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy
- Active pharmacist license, with willingness to obtain licenses in TX, TN, AL, and PA
- Minimum of 2 years of experience in HEDIS-related medication adherence work or quality improvement
- Strong knowledge of HEDIS measures related to medication adherence
- Proficiency in using EHR systems and data analysis tools
