Job Summary
Seeking a detail-oriented Clinical Pharmacist - TX, TN, AL, PA, this full-time remote position will manage HEDIS medication adherence initiatives, analyze data to improve patient outcomes, and collaborate with healthcare providers to enhance adherence strategies.
Key responsibilities
- Lead efforts to improve HEDIS medication adherence and analyze related data to identify gaps in care
- Conduct medication therapy management sessions to address barriers to adherence and educate patients
- Monitor HEDIS performance metrics and provide updates to leadership on progress towards goals
Required qualifications
- Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy
- Active pharmacist license, with willingness to obtain licenses in TX, TN, AL, and PA
- Minimum of 2 years of experience in HEDIS-related medication adherence work
- Strong knowledge of HEDIS measures related to medication adherence
- Proficiency in using EHR systems and data analysis tools
