Job Summary
To improve medication adherence rates among patients, the remote Clinical Pharmacist - TX, TN, AL, PA Licensed will manage HEDIS quality measures, conduct medication therapy management, and analyze adherence data while ensuring compliance with Medicare quality measures.
Key Responsibilities
- Lead efforts to enhance HEDIS medication adherence and analyze related data to identify care gaps
- Conduct medication therapy management sessions to educate patients and resolve adherence issues
- Monitor HEDIS performance metrics and provide updates to leadership on progress and recommendations
Required Qualifications
- Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy
- Active pharmacist license with willingness to obtain licenses in TX, TN, AL, and PA
- Minimum of 2 years of experience in HEDIS-related medication adherence work
- Strong knowledge of HEDIS measures related to medication adherence
- Proficiency in using electronic health records (EHR) and data analysis tools
