Job Summary
To enhance medication adherence and improve health outcomes, the remote Clinical Pharmacist - TX TN AL PA Licensed will lead HEDIS quality initiatives, conduct medication therapy management, and analyze data to implement targeted interventions in a full-time capacity.
Key Responsibilities
- Lead efforts to improve HEDIS medication adherence and analyze related data to identify gaps in care
- Conduct medication therapy management sessions to address barriers to adherence and provide patient education
- Monitor HEDIS performance metrics and generate reports to assess intervention effectiveness
Required Qualifications
- Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy
- Active pharmacist license, with willingness to obtain licenses in TX, TN, AL, and PA
- Minimum of 2 years of experience in HEDIS-related medication adherence work or quality improvement
- Strong knowledge of HEDIS measures related to medication adherence
- Proficiency in using EHR systems and data analysis tools
