Clinical Support Program Manager
Job Summary
Driving operational design and execution, the full-time Clinical Support Program Manager will oversee the scaling of innovative care delivery models and support the migration of electronic medical records while working in a hybrid environment.
Key Responsibilities
- Lead the operationalization and expansion of DaVita Medical Home and DaVita Care Central programs, ensuring effective integration of primary care providers into dialysis centers
- Collaborate with IT and product management to define operational requirements and support the transition of clinical workflows into the Epic platform
- Coordinate cross-functional governance and develop standard operating procedures for provider lifecycle management and billing compliance
Required Qualifications
- Bachelor's degree in Healthcare Administration, Business, Nursing, Public Health, or a related field
- 4-7+ years of experience in healthcare operations, clinical program management, or healthcare technology implementation
- Proven knowledge of value-based care models and Medicare care management programs
- Experience with EHR/EMR platforms such as Epic, CWOW, or OneView
- Strong skills in process engineering and development of standard operating procedures
