Clinical Support Program Manager
Job Summary
Driving the operational design and scaling of innovative care delivery models, the full-time Clinical Support Program Manager will oversee the integration of primary care providers into dialysis centers, support the migration of electronic medical records, and enhance operational workflows across clinical systems in a hybrid work environment.
Key Responsibilities
- Operationalize and expand DaVita Medical Home (DMH) and DaVita Care Central (DCC) programs from pilot clinics to national scale
- Partner with IT and product management to define operational requirements for clinical documentation and ensure seamless data flow across systems
- Coordinate with cross-functional teams to establish compliant billing protocols and manage operational prerequisites for Medicare Fee-For-Service programs
Required Qualifications
- Bachelor's degree in Healthcare Administration, Business, Public Health, or a related field
- 5-7+ years of experience in healthcare operations, clinical program management, or healthcare technology implementation
- Proven understanding of value-based care models and Medicare care management programs
- Experience with EHR/EMR platforms such as Epic, CWOW, or OneView
- Strong skills in strategic problem-solving and cross-functional leadership
