Job Summary
Seeking an experienced and compassionate Cognitive Nurse Practitioner or Physician Assistant, the full-time remote provider will deliver comprehensive telehealth care to patients with Alzheimer's disease and other dementias, collaborating with a multidisciplinary team to manage cognitive disorders and support patient education and care planning.
Key responsibilities
- Conduct comprehensive telehealth visits for patients with cognitive and memory disorders
- Collaborate with interdisciplinary team members to create and implement individualized dementia care plans
- Provide education and support to patients and caregivers to enhance care quality and reduce distress
Required qualifications
- Active, unrestricted NP or PA license and completion of an accredited Nurse Practitioner or Physician Assistant program
- Minimum of 2 years of clinical experience as an NP or PA within geriatrics or neurology
- Strong preference for direct experience in dementia and cognitive care
- Proficiency with telehealth technologies and EHR documentation
- Commitment to continuous learning and evidence-based practice in dementia care
