Colorado Licensed Clinical Pharmacist

Job Summary

To enhance patient care in a remote setting, a Colorado Licensed Clinical Pharmacist will provide integrated support to patients and physicians, focusing on medication management, adherence, and evidence-based therapy optimization in a full-time role.

Key responsibilities

• Manage medication therapy through assessments, reconciliations, and adherence counseling while collaborating with interdisciplinary care teams
• Monitor and evaluate medication regimens to ensure compliance with clinical criteria and identify opportunities for drug therapy optimization
• Participate in quality improvement initiatives and provide recommendations for medication cost savings while supporting workflow optimization efforts

Required qualifications

• Doctor of Pharmacy (PharmD) degree from an accredited pharmacy school
• Current Colorado pharmacist license
• Certification in a relevant field of practice or completion of a postgraduate residency with direct patient care experience
• At least one year of clinical service provision under a collaborative practice agreement or protocol
• Completion of required continuing education to maintain Colorado state pharmacist license
