Job Summary
Providing telephonic care for patients, the part-time Colorado Licensed Telephonic Nurse Practitioner will manage care coordination, perform assessments, and deliver quality care remotely, requiring evening, night, and weekend availability.
Key responsibilities
- Manage telephonic care and coordination among patients, caregivers, and healthcare providers
- Perform comprehensive assessments and document findings in compliance with regulations
- Coordinate care transitions and monitor patient needs to adjust care plans as necessary
Required qualifications
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Active and unrestricted Nurse Practitioner license in Colorado
- Active Nurse Practitioner certification through a national board in relevant specialties
- 3+ years of clinical experience as an Advanced Practice Clinician
- Current DEA licensure/prescriptive authority or ability to obtain post-hire
