Providing telephonic care for patients, the part-time Colorado Licensed Telephonic Nurse Practitioner will manage care coordination, deliver quality healthcare services, and perform comprehensive assessments remotely, requiring evening, night, and weekend availability.
Key responsibilities:
- Manage telephonic care by coordinating with patients, caregivers, and healthcare providers across various settings

Required qualifications:
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Active and unrestricted Nurse Practitioner license in Colorado

- Minimum of 3 years of clinical experience as an Advanced Practice Clinician
- Proficient in using electronic medical records for documentation
