Colorado Telephonic Care Navigator

Job Summary

Working remotely in Colorado, the full-time Colorado Telephonic Care Navigator will conduct telephonic outreach to members, educate them on care gaps, and assist with accessing community resources while maintaining adherence to performance metrics.

Key responsibilities

• Completes telephonic outreach to members utilizing various outreach modes, including inbound calls
• Educates members on gaps in care and assists with scheduling provider appointments
• Links members to community resources and ensures access to primary care providers

Required qualifications

• High School Diploma / GED or equivalent work experience
• 1+ years of call center and/or telephonic customer service experience
• 1+ years of professional experience in an office setting using telephone and computer
• 1+ years of healthcare/insurance or social work/community outreach experience
• Ability to work full-time, Monday - Friday, with flexibility for 8-hour shifts
