Job Summary
Supporting individuals in their daily living skills, the part-time Community Habilitation Specialist will assist participants with self-care, socialization, and community engagement while working remotely.
Key responsibilities
- Assist participants with self-care tasks and daily living skills as outlined in their Individualized Life Plan
- Safely accompany participants to community resources to promote socialization and independence
- Participate in relevant meetings and training sessions to support the development of Individualized Service Plans
Required qualifications
- High School Diploma or High School Equivalency
- Participation in OPWDD community-based training program
- Ability to lift, push, pull 60 pounds without assistance and climb stairs
- SCIP-R Certification/Training
- CPR/First Aid certification
